I believe they're like River in that they were 'made' to do what they do, I don't think they're cyborgs. When River escaped Blue Hands were deployed to retrieve her. The books described a female pair of Blue Hands and when one was killed the other couldn't function. It's possible River was meant to be a Blue Hand agent and the reason she is the way she is, is that she didn't complete the program and get joined with a partner. Just speculation on my part, but it fits.
